Description

If you are an NP passionate about making a positive impact on the health and welfare of seniors, joining this innovative healthcare company could be a great fit. You'll deliver high-quality care to patients as part of a multidisciplinary health and wellness team, working in a clinic setting and visiting patients at home. Two years of experience with a geriatric panel is required.

In-home assessments, chronic disease management, physical exams, medication reviews, vital checks, lab draws, and wound care and depression screenings are among the facets of care you will provide in this relationship-oriented role.
