Shared call among all providers; RN call center handles routine calls
Manageable patient census of 10 – 14 patients per day
Focus on value-based care for complex geriatric patients aged 65 and older
Experience with HEDIS measures and HCC coding highly valued
eClinicalWorks EMR experience preferred
Board-certified or board-eligible; DEA required
