Hybrid role: approximately 50% direct patient care and 50% clinical leadership
Monday – Friday, 8:30 am – 5 pm schedule
Full-time or part-time options available; minimum 3 days per week
Care delivered via telemedicine and in-home visits using a value-based care model
Lead and supervise a team of Nurse Practitioners as Medical Director
Focus areas include symptom management, goals-of-care, and advance care planning
Active California license and board-certified status required; DEA required
Experience in palliative care or clinical leadership strongly preferred
